Work Day Indemnity and Release Form
Twin Oaks Presbyterian Church
I/we, the undersigned, wish to voluntarily participate in the Work Day 2025.
In consideration for being permitted to participate in Work Day, in the cities of Twin Oaks/Valley Park, the state of Missouri, and in the United States of America, beginning the 26th day of April, 2025, I/we, fully recognize dangers and hazards inherent in Work Day, and any related transportation, including personal injury, property damage, or wrongful death, as well as the unknown dangers and hazards which may arise in the course of my participation in Work Day, do hereby voluntarily:
Agree, for myself, my heirs and my personal representative, to defend, hold harmless, indemnify, release and forever discharge, to the broadest extent allowed by law, Twin Oaks Presbyterian Church, it’s trustees, officers, employees, agents, insurers, successors, assigns, from and against any and all claims, demands, actions, or causes of action on account of any damage to real or personal property or any personal injury or death that may result from my participation in the above Work Day.
I/we have read this release, I/we understand it fully, I/we understand that it is legally binding, and I/we understand that, among other things, I/we am/are agreeing to indemnify Twin Oaks Presbyterian Church, for injuries, damages or losses I may cause and giving up rights to sue Twin Oaks Presbyterian Church for injuries, damages or losses I/we may incur.
